Bilateral sinus lift and nasal lift are the selected procedures for this big atrophy. We used puros CORTICAL as the allograft combined with PRF. As the occlusive membrane we used Biomend.
n a posterior surgery we will place six implants ante lateral posterior zone.
We lifted the right maxillary sinus floor with a lateral approach. During the osteotomy we found the posterior superior alveolar artery. We avoided it working inferiorly.
We created a absorbable wall with Collacote membrane and we filled with Puros Allograft mixed with PRF. We covered the graft and the window with PRF membrane.
In this particular case we placed an implant in a previous BoxTechnique horizontal reconstruction. Two previous surgeries were performed trying to achieve osseointegration but failed the both of them.
The defect was treated with BoxTechnique filled with Puros.
After six months we founded a very nice ridge that allow us the implant placement with very nice stability due to the quality of the bone.

The new bone formation allowed to instal the sixth implant. At this moment the case is showing the six upper implants connected with a nice tissue healing . We are working in the final fixed prosthesis .

AT THE CT SCAN A RETAINED CENTRAL INCISOR APPEAR VERY NEAR FROM THE NASAL FOSSA. THE INCISOR IS EXTRACTED AND THE REMAINING ALVEOLAR RIDGE IS REBUILD WITH A BOX-TECHNIQUE™. WE USED CORTICAL PUROS AS A BIOMATERIAL. ON TOP OF THIS A PERICARDIUM MEMBRANE WAS PLACED.
